The Care Management Program
|

Care Management is a free and voluntary program within Denver Health Medical Plan (DHMP) and Medicaid Choice. Our program provides both Health and Wellness Coaching and Disease Management. Through these avenues we are able to offer preventative health education and support as well as identify and manage members who have chronic diseases. Health Coaches help individuals to assess their readiness to change and when ready, provide members with information and skills to facilitate health behavior change. The overall goal of the Care Management Program is to improve health outcomes by motivating individuals to make change and be more active in their care.
This program is available to any DHMP member including Plan A, Point-of-Service, Medicare Select, Medicare Choice and CHP+. We are also available for those members with Medicaid Choice.
