The Chronic Disease Self-Management Program is a preventative evidence based program developed at Stanford University. This workshop is offered for two hours, once a week, for six weeks, in community settings and intended to actively engage people with chronic conditions and encourage self-management of illness. Implementation of the program requires two trained leaders for each six week workshop. Facilitators follow a scripted Leaders Manual to ensure reliability of the program and process. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
Each participant in the workshop will receive a copy of the companion book, Living a Healthy Life with Chronic Conditions, 3rd Edition. This workshop will be offered in both English and Spanish.
