Conversation Maps® - Diabetes Education – 4 week group
Merck, in collaboration with the American Diabetes Association and Healthy Interactions, created an interactive set of conversation tools intended to engage small groups of people with diabetes in an open and engaging discussion about their diabetes. The content of the Conversation Maps® meet ADA Recognition criteria for a complete DSME curriculum. While part of the purpose of the Maps is to educate participants about some of the basics of diabetes, the sessions will allow participants to ask questions about what they are most interested in learning and allows them to learn from the knowledge and experiences of others. Maps take diabetes education further than the traditional models of teaching and is a valuable way for adults to learn, and ultimately change their behaviors to become better self-managers of their diabetes.
Our Health Coaches have been trained to facilitate Conversation Maps® and are excited to offer this unique approach to diabetes education to our patients. The Care Management team would eventually like to train other health care professionals and care providers to use the tools in their areas of practice. This would allow more of our patients the opportunity to experience the benefits of this tool. Merck is currently in development of other Maps including asthma and other chronic health conditions. Small groups utilizing Conversation Maps® are conducted in both English and Spanish.
