What is “Health Coaching”?

Health Coaching is a way to help individuals self-manage their illnesses and conditions, especially those with chronic conditions.

It’s a collaborative partnership that focuses on what people can do to improve their health and promotes individual responsibility for their own health management.

Health Coaching provides motivation, encouragement and compassion to individuals as they address chronic health care needs or lifestyle changes. Coaching goals are strengths-based, patient-centered and directed by the individual.

Health Coaching at Denver Health

Our Health Coaches have diverse educational backgrounds and offer a wide range of clinical expertise including Clinical and Health Psychology, Nursing, Public Health and Social Work.

Our Health Coaches are specially trained in motivational interviewing and skilled at helping guide people into action. Coaches employ a diverse array of assessment, psychological, and behavioral change tools to empower individuals to make change.

Our Health Coaches are integrated into our Community Health clinics offering on-site sessions for individuals and groups. This unique approach is intended to compliment activities and interventions offered by the care team of providers within the patient-centered medical home. Clinic integration promotes face-to-face interaction and assists providers 'in the moment' with referrals and needs.
