
Child Health Plan Plus (CHP+) is a low-cost health plan for children from birth through age 18. Children who are approved for CHP+ must enroll in a health plan. Children who live in Denver, Jefferson, Arapahoe, or Adams county can enroll in Denver Health Medical Plan, Inc. (DHMP) as their health plan.
As a member of DHMP, your child can get care at any of Denver Health¹s 8 health centers throughout metro Denver as well as 12 Denver Health school-based health centers. In addition, CHP+ members enrolled with DHMP pay no copays for covered visits and medicines and have expanded benefits including eyewear and hearing aids.

How can I sign up my kids?
First, make an appointment with an Enrollment representative by calling 720-956-2300. You can download the application below and bring it with you to the appointment, if you'd like. Or the enrollment representative can fill it out during your appointment. Below is a list of the items you will need for this appointment. When you leave this enrollment appointment you will know what program your children are qualified to join.
